Women have more a2 receptors on thigh fat cells compared to abdominal fat cells. This makes thigh fat more difficult to mobilize during weight loss. Increasing local concentration of aminophylline at the fat cell stimulates lipolysis and can lower the lipolytic threshold of thigh fat cells below that of abdominal fat cells. As thigh fat accumulates in women, the distended fat cells can put tension on subdermal connective tissue pulling down on the surface of the skin to form dimpling. Some women are distressed by this process and seek surgical solutions like liposuction. We describe a medical treatment to accomplish the same goal.

Twelve women who considered their thighs to be undesirably fat and dimply participated in the study. A CBC and blood chemistries were done at the beginning and end of the study. Patch testing was done at the end of the study, and a theophylline level was obtained one hour after 2% aminophylline cream had been applied to both thighs. The 2% aminophylline cream was applied to one thigh and the same cream base (prepared by ABIC International Consultants) to the other thigh 5 days a week for 6 weeks. The order of the thighs was assigned randomly and the study was double blinded. Weight and thigh girth were measured weekly. Results were analyzed by paired I test.

Eleven women completed the study. Theophylline levels were undetectable in the peripheral blood, skin patch testing showed no evidence of sensitization, and CBC's and chemistries showed no toxicity. Although group weight remained stable, there was wide individual variation - I ± 2.1 kg (mean ± S.D). All eleven women lost more girth on the aminophylline treated thigh than the placebo treated thigh - 1.51 ± .8 cm (p<.Ol). The women liked the cream base and the results. We conclude that 2% aminophylline cream applied topically causes regional fat loss from the thighs of women."
